Community Connector

Position Summary: The Shames JCC is seeking a Community Connector to help older adults manage loneliness, access to services, or other challenges by linking them to appropriate resources in the community and helping to organize transportation to those resources.

Key Responsibilities: 

  • Intake and Needs Assessment:  Assess the unmet needs of older adults who seek assistance.
  • Case Management and Referrals: Provide case management services, including information and referrals to other organizations and resources, and provide ongoing follow-up and monitoring.
  • Transportation Planning and Coordination: Develop and implement transportation plans, coordinating with drivers, volunteers, and other service providers.
  • Community Building:  Seek opportunities to foster connections among older adults and facilitate their engagement with the broader JCC community.

Qualifications:

  • Bachelor’s or Master’s degree in Social Work (BSW or MSW) is preferred.
  • Experience working with older adults and/or individuals with disabilities is highly desirable.
  • Familiarity with public transportation, private transportation services, and transportation options for seniors.
  • Awareness of and willingness to explore relevant resources for seniors.
  • Strong case management skills, including assessment, planning, and advocacy.
  • High EQ, and excellent communication, interpersonal, and problem-solving skills.
  • Self-motivated and comfortable working independently in moving from issue identification to proposing solutions.
  • Fluency in multiple languages (Spanish ideally) is helpful.
  • Proficiency in using computer software and databases.
  • Strong organizational and time management skills.

Salary: $40,000 for part time role (approximately 25 hours per week)
